5)    Is there a contractor’s license number on all contracts subcontracts calls for bid and other forms of advertising?  

 

 

6)    On the contract you signed is the wording “contractors are licensed by the Contractors state license board”?  

 

 

7)    Start date of job?  

 

 

8)    Finish date of job?

 

 

9)    Amount of contract?

 

10)                       Did you receive notice to owner regarding lien provisions?

 

11)                       On the contract was start dates and finish dates for each project?

 

12)                       Did the contractor take more than 10% down or if the contract was over $10,000 did the contractor take more than $1000 deposit?

 

13)                       Did the contractor get you to sign change orders?

 

 

14)                       Important stuff:

Did the contractor at any time fail to proceed with your project or operation with reasonable diligence causing you material injury?  

 

 

 

15)                       Did the contractor leave the job not come back to finish?

 

 

16)                       Did the contractor willful disregard of plans and specifications?

 

 

17)                       Did the contractor fail to complete the job in a good and workmanlike manner?

 

 

18)                       Did the contractor fail to complete a project for the price stated in the contract?

 

19)                       Amount you paid?

 

 

20)                       Amount still owed?

 

 

21)                       Other stuff: Was this a joint venture or one contractor?

 

 

22)                       Was the licensed contractor at the job at all times?

 

 

23)                       Is the job finished or is the contractor still working on the job?
